Acorn Community Church is passionate about providing a safe and fun environment while individuals enter our premises which is why our volunteers have to go through a DBS check and Safeguard training.

as this has to be a factual account and include the following information:

• Names of anyone involved

• Date, time, and location of concern/disclosure

• What the person(s) said or did which caused concern

• Use the person(s) own words

• What you said

• How they represented (were they upset, angry etc.)

• Record consent to share information - if appropriate

• Sign and date it (including any additions)
